How not to crimp kembla

I was called into a job yesterday where the customer could smell gas.
The customer had called back the installing gas fitter twice, and they said there was no leak.( also had paid them )  ???
As you can see from the pic, the kembla 16x1/2 fitting is clearly crimped incorrectly, i would say that the crimp jaw has not been positioned completely over the crimp ring and then they have had a second go at it !!!! ??? ??? ??? ??? ??? ???

As a result the tail inside the fitting has been crushed and would never seal, but this installation has been in service for over a year!
the installing gas fitter clearly never tested the whole system or they would have picked it up.
The fitting was the last crimp before the metal flexi hose to a dl850 escea

The installation was certed and i found it on pgdb web site, and know who the gas fitter is....not that we will ever be able to do that in the future!!!!! :( :( :( :( :(
 The owners are now going the building co, and in turn they will go the gas fitter,

what do you all think? is this a notifiable issue? the leak was 0.01 kpa per minute on a wp test.

hi guys/Bowtieboy, just shows how important testing is! and testing is so easy. looks like the wrong size crimp jaws were used,cheers

Is there a crimp jaw size smaller than 16mm?
I think the guy pulled the button on his crimper with only half of the jaw over the ring and stopped, saw that it was wrong and had a second go.
What gets me is it looks like its a pexal crimper that has been used  ??? as there is 2 lines in the crimp, kembla doesnt have that. ::)

yeh does look like a pexal/rifeng crimper.
Have seen these crimps been done wrong with the correct tool when the jaws haven't been put round the fitting fully and have started the crimp and the jaws dig into the copper ring and leave a raised section.
